Most doctors begin the treatment process by prescribing their patients ED medications. But if that isn’t successful, men can turn to vacuum therapy, injections, or penile implant surgery. If you want to learn about all these treatment options, just keep reading!
How to Know if You Have Erectile Dysfunction
The main symptoms that will reveal you have erectile dysfunction are changes in your erections. This could mean your erections are not firm enough for intercourse or you are unable to keep an erection long enough for sex. You could start getting erections that aren’t as girthy as they used to be or are at a different angle than they once were. Some men may be unable to achieve erections at all, and some may even experience less sexual desire.
What are the causes of Erectile Dysfunction?
In this, there is no actual ED. Early morning erections are normal. The common causes are uncomfortable surroundings, work stress, marriage stress, lack of interest with the partner.
- Arteriogenic: There is reduced blood flow into penis while erection due to arterial block. The common causes are Diabetes or Cholesterol deposition in the blood vessels.
Syn. Venous leak or Venogenic ED or Penile venous insufficiency. The veins of penis become leaky and are unable to hold the blood inside the penis. So, erection happens, but penis becomes soft even before ejaculation. The common causes are trauma like Penile fracture.
- Neurogenic: The nerves for erection are damaged, so patient cannot achieve an erection. The commonly seen after Radical prostatectomy surgery for prostate cancer and congenital nerve abnormality
How to diagnose Erectile dysfunction?
The best test for evaluating ED is Penile Doppler Ultrasound test.
In this, we inject 30 to 60 mg papaverine into the corporal bodies of the penis. This increases the blood flow into the penis. The normal readings are Peak systolic velocity of more than 30cms/sec and end diastolic velocity of less than 3cms/sec. If the Peak systolic velocity is less than 30cms/sec, it indicates arterial blockage as cause of ED and if the end diastolic velocity of more than 3cms/sec, it means Venous leak or Venogenic impotence. The penile doppler ultrasound study is normal in Psychogenic ED and Neurogenic ED.
Erectile dysfunction treatment in India: Options
Using Medications to Treat Erectile Dysfunction
ED medications work by making nitric oxide more effective. Nitric oxide is the chemical you produce that causes the muscles in your penis to relax. The result is more blood flow to the penis to help you achieve an erection.
There are several options for medication on the market. They vary based on how long it takes for them to work and how long the effects last before they wear off. Every medication comes with different side-effects and potential interactions with other medications. Only your doctor can prescribe you the medication that’s best for you.
Vacuum Therapy for Erectile Dysfunction
If you find that ED medication is not working for you, the next treatment option for you could be vacuum therapy. With this treatment, blood will be sucked from your body and channeled to your penis to help you get an erection. This is successful for many men. However, many men also find this treatment uncomfortable.
This treatment is common for men who have had a radical proctectomy for prostate cancer and now need penile rehabilitation because of it. It’s also common among patients with diabetes or cardiac problems.
Injections for Treating Erectile Dysfunction
Getting injections is another option, but not a very popular one. Although it’s effective, it’s inconvenient and can be painful. These injections go directly to the penis. Your doctor will begin by teaching you how to do them and then you will be able to administer them yourself before sexual intercourse.
You are also limited to only 12 injections per month. Patients who choose this method need to alternate which side of the penis they inject. Unfortunately, if you continue using injections over a long period of time, you could develop fibrosis in your penis.
Penile Implant Surgery
Sometimes a doctor may prescribe penile implant surgery if medications have been unsuccessful. Although it might seem scary to consider surgery so soon, it's one of the most effective treatments for erectile dysfunction.
A penile implant is a prosthetic device attached to your penis and scrotum. Even after you’ve chosen this route for ED treatment, you have options. You can choose a two or three-piece inflatable implant. There are also semirigid penile implants.
Inflatable Penile Implants
With a two-piece inflatable implant, you have a cylinder in your penis shaft as the first piece. The second is a combined fluid reservoir and pump. The reservoir-pump piece goes in your scrotum, and the fluid is inside the pump. The three-piece implant is similar. But instead of the reservoir and pump being combined, they are two separate pieces. So in three-piece implant, there is a penile cylinder, a scrotal pump and a retropubic reservoir.
With either type of inflatable implant, you can pump fluid from the reservoir to the cylinder. This will imitate the look and feel of an erection. Squeezing the pump is easy, but not so easy that you need to worry about accidentally inflating the implant at the wrong time.
Inflatable penile implants are easy to conceal. When you’re not trying to achieve an erection, your penis will still feel natural. You may not even be aware that it’s there while your penis is flaccid. Really the only sign of an implant is the incision your surgeon makes to perform the surgery. This is a small incision at the base of the penis. This incision mark will totally disappear after few months.
Men who choose inflatable implants are sometimes worried about how it will affect their own ability to get aroused. Fortunately, your penis will still have the same sensation. It won’t affect your sex drive either.
Semirigid Penile Implants
Unlike inflatable implants, semirigid implants are always the same size and stiffness. These implants are made of two rods that are flexible enough to be controlled. Most of the time they will be facing down. When you’re ready to have sexual intercourse, all you have to do is straighten the rods by pushing them up. This will emulate a natural erection.
The design of malleable implants is simple and has a very high success rate. These implants are less expensive than their inflatable counterparts. The biggest advantage of malleable implants is that they have low chances of infection and so they are best implants for Diabetic patients.
The Penile Implant Surgery Process
Penile implant surgery isn’t a very lengthy procedure. It should take between 45 minutes and two hours. You’ll be under some type of anesthesia during surgery. You’ll have an IV, and you may need a catheter. Before the surgery, the doctor will shave around the penis to make the procedure go more smoothly. The surgeon will then make an incision.
Next, your surgeon will stretch the spongy tissue that’s in your corpora cavernosa. This is the tissue that will fill up with blood when you’ve achieved an erection. Now is the time for the actual implant. The surgeon will insert all the pieces of your chosen type of implant. Once they are done, the surgeon sews up the incision to remove any trace of the surgery.
The recovery process from this surgery lasts a few weeks. You may experience some pain even with prescription medication. But if you take it easy and avoid sexual intercourse for a few weeks, you should have a strong chance of success and an easy recovery.
One of the biggest things that can go wrong post-surgery is getting an injection. Because of this, you might get an antibiotic to help you avoid infection.
You should keep in mind that getting penile implant surgery is irreversible. This makes it even more important that you carefully consider the risks involved.
